Personal Data Protection Information Notice
I hereby acknowledge that I have read this Information Notice and accept, declare, and undertake in advance that, within the scope of the Law No. 6698 on the Protection of Personal Data, I give my explicit consent for my personal data to be collected, recorded, processed, and stored by MagiClick in accordance with the principles set forth in the law.
Pursuant to Law No. 6698 on the Protection of Personal Data and the Regulation on the Processing of Personal Health Data and Ensuring Privacy, I accept, declare, and undertake that my personal data, my special categories of personal data, and my health data — including all personal data that identify or may identify my identity, provided by me verbally, in writing, and/or electronically — may be processed by MagiClick for the purposes of candidate selection for the execution of an employment contract, the signing and performance of the employment contract, the provision and planning of healthcare services, and may be shared with other persons and institutions where such sharing is required under the relevant legislation.
I hereby give my consent to the processing of my personal data, special categories of personal data, and health data as defined under Law No. 6698 and the Regulation on the Processing of Personal Health Data and Ensuring Privacy, within the scope detailed below.
The personal data subject to processing include, but are not limited to:
- Data that clearly belong to an identified or identifiable natural person and are processed wholly or partially by automatic means or non-automatic means as part of a data recording system; identity information contained in documents such as driver’s licenses, national identity cards, and passports, including name and surname, Turkish Republic Identification Number, nationality, mother’s and father’s name, place and date of birth, gender, and tax identification number.
- Social Security Institution (SGK) number, signature information, vehicle license plate number, professional certificates, and similar information.
- Data that clearly belong to an identified or identifiable natural person and are processed wholly or partially by automatic means or non-automatic means as part of a data recording system; contact information such as telephone number, address, email address, fax number, and IP address.
- Personal data relating to records and documents obtained during entry into and presence within physical premises, including CCTV recordings, fingerprint records, photographs, audio and video recordings taken for use on the website, company social media posts, corporate promotional films, training sessions, presentations, brochures, proposals, and security checkpoints.
- Data that clearly belong to an identified or identifiable natural person, including photographs and camera recordings (excluding records falling within the scope of physical premises security information), and data contained in documents that are copies of documents including personal data.
- Data that clearly belong to an identified or identifiable natural person and are processed wholly or partially by automatic means or non-automatic means as part of a data recording system; special categories of personal data specified in Article 6 of the Law on the Protection of Personal Data (e.g., health data including blood type, biometric data, etc.).
- Data that clearly belong to an identified or identifiable natural person and relate to the receipt and evaluation of any requests or complaints submitted to the organization.
I hereby confirm that I have read and accepted the above text and my rights regarding the Protection and Processing of Personal Data as published on MagiClick’s website.
I will submit my request for the deletion of my personal data collected by Magiclick under KVKK by requesting and completing the KVKK Information Request Form through the contact page.
Kişisel Verilerin Korunması Hakkında Aydınlatma Metni
İşbu Bilgilendirme Yazısını okuduğumu ve 6698 sayılı Kişisel Verilerin Korunması Kanunu kapsamında, kişisel verilerinin MagiClick tarafından işlenmesine yasadaki esaslar çerçevesinde toplanmasına, kaydedilmesine, işlenmesine saklanmasına peşinen izin verdiğimi kabul, beyan ve taahhüt ederim.
6698 sayılı “Kişisel Verilerin Korunması Kanunu” ve “Kişisel Sağlık Verilerinin İşlenmesi ve Mahremiyetinin Sağlanması Hakkında Yönetmelik” gereğince, kişisel verilerimin, özel nitelikli kişisel verilerimin, sağlık verilerimin işlenmesine, tarafımca sözlü/yazılı ve/veya elektronik ortamda verilen kimliğimi belirleyen veya belirlemeye yarayanlar da dahil olmak üzere her türlü kişisel verimin, MagiClick tarafından işlenmesine, iş sözleşmesi yapılması amacıyla aday seçimi, iş sözleşmesinin imzalanması ve iş sözleşmesi gereği ile sağlık hizmeti verilebilmesi ve bu hizmetlerin planlanması amacıyla, ilgili mevzuatlar kapsamında paylaşımın gerekli olduğu diğer kişi ve kuruluşlar ile paylaşılmasına; kişisel veriler, özel nitelikli kişisel veriler, sağlık verilerimin 6698 sayılı “Kişisel Verilerin Korunması Kanunu”nda ve “Kişisel Sağlık Verilerinin İşlenmesi ve Mahremiyetinin Sağlanması Hakkında Yönetmelik”te tanımlanan kapsamda aşağıda detayları verilen kişisel ve özel nitelikli kişisel verilerin işlenmesine muvafakat ettiğimi kabul, beyan ve taahhüt ederim.
- Kimliği belirli veya belirlenebilir bir gerçek kişiye ait olduğu açık olan; kısmen veya tamamen otomatik şekilde veya veri kayıt sisteminin bir parçası olarak otomatik olmayan şekilde işlenen; kişinin kimliğine dair bilgilerin bulunduğu verilerdir; ad-soyad, T.C.Kimlik numarası, uyruk bilgisi, anne adı-baba adı, doğum yeri, doğum tarihi, cinsiyet gibi bilgileri içeren ehliyet, nüfus cüzdanı ve pasaport gibi belgeler ile vergi numarası,
- SGK numarası, imza bilgisi, taşıt plakası, uzmanlık belgesi v.b. bilgiler
- Kimliği belirli veya belirlenebilir bir gerçek kişiye ait olduğu açık olan; kısmen veya tamamen otomatik şekilde veya veri kayıt sisteminin bir parçası olarak otomatik olmayan şekilde işlenen; telefon numarası, adres, e-mail adresi, faks numarası, IP adresi gibi bilgiler
- Kimliği belirli veya belirlenebilir bir gerçek kişiye ait olduğu açık olan; kısmen veya tamamen otomatik şekilde veya veri kayıt sisteminin bir parçası olarak otomatik olmayan şekilde işlenen; fiziksel mekana girişte, fiziksel mekanın içerisinde kalış sırasında alınan kayıtlar ve belgelere ilişkin kişisel veriler; kamera kayıtları, parmak izi kayıtları, web sitesinde, şirketin sosyal medya paylaşımlarında, şirket tanıtım filmlerinde, eğitimlerde, sunumlarda, broşür teklif gibi basılı evraklarda kullanılmak üzere alınan fotograflar, sesler, görüntüler ve güvenlik noktasında alınan kayıtlar.
- Kimliği belirli veya belirlenebilir bir gerçek kişiye ait olduğu açık olan; fotoğraf ve kamera kayıtları (Fiziksel Mekan Güvenlik Bilgisi kapsamında giren kayıtlar hariç), kişisel veri içeren belgelerin kopyası niteliğindeki belgelerde yer alan veriler
- Kimliği belirli veya belirlenebilir bir gerçek kişiye ait olduğu açık olan; kısmen veya tamamen otomatik şekilde veya veri kayıt sisteminin bir parçası olarak otomatik olmayan şekilde işlenen; Kişisel Verilerin Korunması Kanunu’nun 6. maddesinde belirtilen veriler (örn. kan grubu da dahil sağlık verileri, biyometrik veriler vb.)
- Kimliği belirli veya belirlenebilir bir gerçek kişiye ait olduğu açık olan; kısmen veya tamamen otomatik şekilde veya veri kayıt sisteminin bir parçası olarak otomatik olmayan şekilde işlenen; Kuruluşa yöneltilmiş olan her türlü talep veya şikayetin alınması ve değerlendirilmesine ilişkin kişisel veriler
